Going full waterfall on a novel AI problem. Planning the entire project upfront when you do not know if the approach is feasible leads to plans that are wrong from day one. Always validate feasibility with a spike before committing to a full plan.

Going full agile without a clear definition of done. Sprinting without defined success criteria leads to endless iteration. Establish quantitative targets before starting development, even if you adjust them later.

Changing methodology mid-project under pressure. When an agile project falls behind, the instinct is to “lock down the plan” and go waterfall. When a waterfall project hits unexpected complexity, the instinct is to “go agile.” Both transitions are disruptive. Pick your approach, adapt it to your needs, and commit.
